El Shaddai: Ascension of the Metatron is an adventure/platforming game. You play as Enoch, a man that has been sent by God battle seven Fallen Angels and imprison them to save the world from the biblical Great Flood. Enoch must traverse the Fallen's tower and combat the different challenges the Fallen throw in his path before God floods the world and kills humanity.

Armaraos is said to be the Player Character’s friend. There is no hint of the friendship in the game; their friendship is only mentioned by other characters. After completing the section where the player plays as Armaros, Armoros is murdered on screen.
After defeating Armaros it appears that the young girl Nanna dies.
After the player defeats Ezekiel, Nana/Ishtar is shown dying.

Drowning
All of the level “Cry of Armaros” is underwater. The Player Character can fall into the water, but it is a game over before the Player Character hits the water.

The Gale weapon can create explosions.
In Belial’s realm, there are mines that the player must activate and then run from before they explode.
Fire
The powerups for Uriel are fire that you run through, though it is magic fire.
Multiple characters talk about others burning to death. This is not shown on screen.
Ezekiel's level has rings of fire you must jump through.
The level “Tragedy of Baraquel” has fire falling from the ceiling.
After the motorcycle part of Azazel’s Zeal level, the screen is engulfed in fire and the Player Character walks out of it.
The boss Nether Ezekiel uses fire attacks.
When the player reaches higher level so Uriel’s boost power, the Player Character will trail fire behind him.
The boss Azazel’s second form uses fire to fight.
Ghosts
Spirits of the Beloved are enemies that are ghosts that will possess you as an attack.
There are entities in the platforming section after Ezekiel’s lair revealed to be Sauriel’s beloved---souls that are forced to stay with Sauriel for eternity. They can attack and possess the Player Character.

Most of the game involves platforming at heights where you can easily fall.
In Belial’s realm there are several parts where the player is forced to make blind falls during platforming.
Insects
There are enemies called Ramps with thick bodies and spindly legs. They are humanoid, but could be triggering for someone sensitive to insects.
After the player defeats the Fire Nephilim the player character is taken into the sky by a giant insect. The boss Azazel fights with swarms of insects. His second form involves him being swarmed by a huge amount of insects and turning into a giant wasp. In the second form the floor also turns into bugs if you stand on it too long.

After defeating Armaros the first time, a young child is apparently dragged to Hell. She is saved by the Player Character.

In Belial’s realm there are spinning sword blades that impede the player during platforming sections.
