Binding of Isaac follows a young boy whose mother has begun hearing "voices from above" that tell her that her son is full of sin and she must "cleanse" him. The game chronicles Isaac coping with and escaping from his mother's increasingly violent and eventually homicidal abuse.

Animal Abuse

Some animals killed for self defense, some do not attack but need to be killed in order to progress.

Animal Death

Some items reference animal death, such as the Dead Cat and Max's Head. The cat and the dog are the Player Character's pets.

Avians

There is a bird power-up that can be used during combat and will attack enemies.

Blood

Blood is extremely common in the game. Some power-ups and enemies spray blood as a weapon. Boss monsters explode in a burst of blood when defeated and some enemies leave bloodstains when defeated.

Body Horror

Some power-ups cause extreme distortions to the player character’s face or body including growths and detachment of the head from the body.

Bones

Some enemies have skeletal features or features that resemble bones.

Brainwashing

It could be seen that the Player Character's mother was brainwashed into believing he was evil.

Canines

There is a severed dog's head that can be found in the game.

Cannibalism

The Yum Heart item references eating a heart.

Claustrophobia

Some inter-level scenes depict Isaac trapped in a locked chest.

Darkness

The curse of darkness attack in the game limits the player's view of the map and ability to see enemies.

Dead Bodies

The best friend item leaves a 'corpse' that enemies attack instead of Isaac, and the playable character.

Death of Family/Friends

The items Dead Cat and Max's Head reference the death of the Player Character's pets.

Dehumanization

In the beginning sequence Isaac is shown to be stripped of things considered 'sinful' including clothes and access to the outside world.

Disease

Some power-ups cause the player character to appear diseased and generate vomit-green projectiles.

One boss is the representation of Plague of the four horsemen.

Disruptive Home Life

The story of the game is Isaac’s mother believes God has told her to kill her son and Isaac therefore hides in the basement/a chest.

Emotional Abuse

Isaac is stripped of anything enjoyable or 'sinful' in the opening sequence by his mother. She then attempts to murder her.

Explosions

Bombs are frequently used in the game and explode both visually and audibly.

Some power-ups cause the player character’s shots to explode on impact.

Eye Horror/Eye Trauma

Some power-ups cause distortion or loss of the play character’s eyes.

The boss Peep loses his eyes as the battle progresses.

Felines

Isaac has a pet cat that is seen multiple times in the game. The cat is not hostile.

Fire

Campfires in the game can harm Issac.

In the Wrath of the Lamb DLC, some fire will shoot projectiles at the player.

Ghosts

Some power-ups cause the player character to gain a ghost-like appearance (pale face, wispy tail instead of legs).

Gore

Many enemies have detached heads or body parts that detach from their body as the battle progresses.

Some enemies attack by ripping open their chests and fire lasers/maggots.

Bosses explode with blood when defeated.

Some pickups are modeled after organs, some change Isaac's appearance to a gory model, some bosses appear mutilated in a way that some organs are exposed, and a lot of blood is shown throughout the game.

Graphic Violence

Bosses will die in violent manners and will show damage graphically, such as losing eyes.

Guns

Many enemies use projectiles within the game.

Heights

Pits are scattered throughout the levels but the player cannot fall into them.

Insects

Flies and maggot-like enemies appear frequently.

Some bosses resemble large worms or maggots.

Mutilation

Some power-ups cause extreme distortions to the player character’s face or body including growths and detachment of the head from the body.

Needles

Some power-ups appear as needles.

Nudity

The Player Character and other characters in the game do not have clothing. No genitalia is shown.

Parental Abandonment

The Player Character's mother suddenly believe he's evil and attempts to kill him.

Psychological Trauma

Inter-level loading pages depict drawings of Isaac’s memories of past events. These include being locked in a chest, being mocked for wearing a wig, being given a gift box full of poop, and having his pants pulled down in front of a girl. All of this is shown to have had an extreme negative effect on the Player Character.

Reptiles

Some pickups in the game appear reptilian.

Self Harm

One of the usable items is a razor, which causes harm in return for a power-up.

A pair of scissors is also usable which detaches the character’s head from their body.

Snakes

Some items appear snake-like, and the bosses Larry Jr. and Pin have no legs but are segmented.

Spiders

Many enemies are spiders or spider-like creatures.

Substance Abuse

Pills appear frequently as power-ups and can cause both positive and negative side-effects.

Suicide

Some items found in the game reference suicide.

Supernatural Evil

Satan appears as a statue and eventually as a boss in the game.

Many power-ups are evoke demonic worship and symbolism.

Torture

Some items found in the game reference torture.

Undead/Zombies

Some enemies can be thought of as zombie-like and the noises they make are zombie-like.

Violence Against Children

Isaac is a child and is put through abuse in the opening sequence, and is the playable character so therefore he can get hurt and die.

Vomiting

Some enemies make vomiting noises or attack through vomit.

Weapons

Knives occasionally appear as power-ups or usable items.
